ABSTRACT

Sudden headache onset may rarely be caused by spontaneous intracranial hypotension (SIH). Other associated symptoms in patients with SIH are nausea, vomiting, vertigo, hearing alteration, and visual disturbance. This case report describes a 43-year-old female diagnosed with SIH who developed diplopia after resolution of an abrupt-onset headache, which was managed with conservative treatments, including bed rest and hydration. She was also diagnosed with secondary right sixth cranial nerve palsy. Although conservative management relieved her headache, the diplopia was not fully relieved. Application of an autologous epidural blood patch successfully relieved her diplopia, even after 14 days from the onset of visual impairment.

ABSTRACT

Horner syndrome is characterized by miosis, partial blepharoptosis and anhidrosis on the affected side of the face. This syndrome develops when the oculosympathetic nerve pathways to the eye and face are interrupted by various causes such as tumor in the brain, intrathoracic region or neck, surgery, drugs, trauma, carotid artery dissection, and others. It is referred to as painful Horner syndrome when Horner syndrome is accompanied by hemifacial pain. Pain is probably related to trigeminal nerve injury. Horner syndrome is a rare complication of thyroidectomy. Here, we report the case of a patient who experienced ipsilateral painful Horner syndrome after total thyroidectomy and unilateral neck dissection for thyroid cancer.

ABSTRACT

Mucormycosis is an uncommon, frequently fatal, opportunistic fungal infection which usually develops in immunosuppressant patients, especially in patients with AIDS, organ transplantation, diabetic mellitus, administration of steroid, chronic illness, severe malnutrition. It is classified into four groups as clinical forms and one of its, gastrointestinal type is an unusual form, believed that infection of the gastrointestinal tract is acquired through ingestion of the ubiquitous fungal spores, the stomach being the most frequently involved site. Especially, gastric mucormycosis, vascular invasive type is frequently fatal. Recently we experienced a case of gastric mucormycosis, vascular invasive type found in a 74-year old male undergoing hemodialysis for chronic renal failure and he was in severe malnutrition. In the course of the admission, unexpectedly, hematemesis was developed. Upper gastrointestinal fiberoscopy was done and we found a ulcerative lesion, geographic in body of stomach and on histologic examination, characteristic findings of nonseptate hyphae with right angle branchings was observed in the ulcer debris with vessel invasion. The patient was treated with systemic antifungal agent, amphotericin B but expired due to massive hematemesis.

ABSTRACT

PURPOSE: IgE was identified by Ishizaka as an antibody that is reacted at Type I hypersensitivity reaction, and has been used in the diagnosis of allergic disease and the differential diagnosis of extrinsic and intrinsic asthma. But there was some confusing in the diagnosis because of the broad range of measured serum IgE. Recently it was reported that serum IgE should be standardized for area and age to give the diagnostic reliability. METHODS: We measured serum IgE in 512 Korean children admitted at Ewha Woman University Mokdong Hospital between March 1991 and March 1994 and studied the relationship between asthma and serum IgE standardized for age in Korean children. RESULTS: 1) The geometric mean of serum IgE in non - allergic children and asthma were increased gradually according to age. The ranges of measured IgE in each group were overlapped in some part. 2) Log IgE increased by age similarly in asthma and non - allergic children, but was higher in asthma children than non - allergic children. Z scores of log IgE in asthma children increased by age, but those in non - allergic children was not changed by age. 3) Log IgE in male was not significantly different to that of female (p>0.05). 4) Incidence of asthma increased by increasing of Z score of log IgE standardized for age(p<0.05). 5) The normal range of serum IgE standardized for age was presented. Asthma was absent in children below the lowest normal IgE level (log IgE-1.5SD). CONCLUSIONS: Serum IgE if standardized by age, would be much valuable for the diagnosis of asthma in children.
